Investigation Summary
At approximately 8:30 p.m. on August 6, 1997, Employee #1 was operating a Crown electric forklift in reverse. He was turning down an aisle to retrieve a pallet when the forklift went out of control. Employee #1 tried unsuccessfully to plug the vehicle by using the forward switch to assist in the braking, and he was thrown forward when the forklift struck a concrete crash post. Employee #1 became pinned between the lift and the post, fracturing his pelvis.
