Employee's fingers amputated in chain and sprocket drive

At 11:20 p.m. on October 28, 1997, Employee #1, a walnut sorter, was about to finish her shift. She reached under the end of the 24 in. wide running conveyor belt to use a paper towel to clean some walnuts off a small shelf for the 8 in. wide conveyor pulley's motor transfer case. The towek became caught and pulled her hand into the unguarded 2 in. wide chain and sprocket drive located behind the 3 1/2 in. long by 5 in. wide by 2 1/2 in. deep transfer case. Her right middle and ring fingers were amputated.
