Investigation Summary
Employee #1 was attempting to assist a coworker by kicking a water hose under a conveyor. While kicking the hole near the machine/equipment frame, Employee #1's pant leg got caught by the the rotating shaft's sprocket. The rotating shaft runs under the bench line and was not guarded.
