SHE LOST PART OF HER RIGHT MIDDLE FINGER AND RIGHT RING FINGER. THE CHAIN AND SPROCKET DRIVE WERE NOT GUARDED. A SERIOUS CITATION AND PENALTY WERE ISSUED ALLEGING A VIOLATION OF 3999(B).
