Investigation Summary

Investigation Nr: 170883987
Event: 06/18/1996
Two employees have finger injuries in cone machines

Employees #1 and #2 worked for Norse Dairy Systems, Los Angeles, CA, an ice cream cone manufacturer. At this location, there were 26 machine operators to run 16 automatic cone baking machines during a 24-hour period. The machines, manufactured by Drumstick, of Columbus, OH, were being operated without guards over the chain and sprocket areas. With her left hand, Employee #1 reached 4 in. into cone machine #3 to clear a cone cake that was hanging outside of the mold. She was extracting her hand and the cone cake when her thumb and fingers struck against the unguarded sprocket chain, amputating her left index finger. Employee #2 reached into cone machine #3 with a gloved hand to clear a cone cake inside the machine near the main sprocket and chain. The fourth digit on his left hand was amputated.
