Employee's thumb amputated in printing machine

Employee #1, of the Los Angeles Times, was attempting to unjam a malfunctioning Crosfield Pagefax II Writer, model PX-0664, by using a posted error code determination. She was reaching behind a metal panel when her left hand became caught between the stationary punch bar and moving film tray, amputating her thumb. The company policy was to contact maintenance for equipment jams in this area. Employee #1 failed to obey company rules regarding maintenance and unjamming equipment.
