Investigation Summary

Investigation Nr: 170633606
Event: 03/27/1997
Employee's arm injured when caught by steel roller

Employee #1, of Precision Specialty Metals, Los Angeles, CA, was observing paper-lined, oil- enhanced, rolled steel sheeting making its way through the machine process, as he had done many times over the past three years. He saw loose liner paper and other foreign matter on top of the sheet metal and, realizing it would mark the light gauge steel and could cause the product to be rejected, grabbed a rag to wipe the steel. As Employee #1 lifted the paper line with his left hand, oil dropped off it and landed in his working area, causing him to slip. He reached out with his right hand to steady himself, and his rag, hand and arm were drawn into the in-running steel roller. Employee #1 fractured a bone in his arm and sustained numerous arm lacerations and tendon damage.
