Employee burned by release of pressurized ammonia

Employee #1, of Union Central Cold Storage, Inc., Los Angeles, CA, was helping to remove a valve housing assembly. When the housing parts were separated, a portion of the line fell and released pressurized ammonia. Employee #1 sustained third-degree burns on his hands, legs, and abdomen. Subsequent investigation revealed that the employer had purged the pressurized ammonia from the lines, and the gauges attached to them read zero. However, a 6 ft section of line had not been depressurized, and the rear pressure check valve was in the 'up' position when it was supposed to be down. Employee #1 also stated that the bolts holding the valve assembly should have been slightly loosened, but they had been completely removed.
