Investigation Summary
An employee was checking the roof of an elementary school gym for leaks. The employee went to the roof, which was surrounded by a 26-in. parapet wall. He attempted to climb the parapet wall to access a second roof area 10 ft above the area he was on. He stood on the parapet wall and grabbed the first row of tiles on the second roof area. The tiles came lose and the employee fell. He sustained a fractured pelvis and was hospitalized. He did not use a ladder to access the second roof area.
