Investigation Summary
Employee #1 was walking through the wash area of the yard. The tanks of tanker trucks are drained and cleaned prior to being released for the next load. The cleaning materials used in the process are either water and detergent, or a caustic soda solution. These products are drained to the asphalt floor area which is slightly slanted towards the center of the wash area drain and the area of over flow fluids. These products never drain entirely because of poor maintenance to the floor area, cracks and potholes are visible through out the area). Employee #1 stated he was talking with the wash area attendant in the middle of the wash area. When Employee #1 turned to leave the area, to obtain supplies from his truck, he slipped on the slippery surface and the grate in center of wash area and fell to the ground. Employee #1 dislocated his left hip and received chemical burns from the mixture of products that were lying on floor. The fluids in area had not drained entirely because of poor drainage due to improper upkeep of area asphalt.
