Investigation Summary
At approximately 9:55 p.m. on October 27, 1993, Employee #1 set up and ran the #2 printing machine. When he attempted to shut the sheet metal guard or cover on the machine's worm gears, he slipped on some lubricating oil that had leaked on the floor and lost his balance. His right middle and ring fingers were caught and crushed in the gears. He was taken to Stacy Medical Center and then to Centinella Hospital in Los Angeles. Serious violation of T8CCR 3944(c) was observed and cited.
