Employee's finger amputated by saw blade

At approximately 7:15 a.m. on February 11, 1994, Employee #1, of the City of Los Angeles General Services Department, was using a Haberle H 350 table saw to cut steel in the fleet maintenance shop. When he attempted to remove a small piece of metal from the saw with his gloved left index finger, the finger and glove became caught in the saw. The finger was amputated. Employee #1 should not have been wearing gloves while operating the table saw. The employer was cited for a serious violation of T8CCR 3384(b).
