Employee's hand lacerated by machine rollers

At approximately 3:30 a.m. on July 2, 1993, Employee #1 was checking the operation of a Flexo Folder Gluer. In order to observe ink that was leaking off, he had to bend down under the rails approximately 4 feet above the floor, get a close look at the moving machinery, and turn back, again bending under the rails to get out. While below the rails on the way out, he slipped on the floor, which had grease, ink, and water on it, lost his balance, and caught his left hand (which held a rag) between the revolving analox rolls and shaft. He was taken to the Norwalk Community Hospital and hospitalized for lacerations of the left hand. General violation 8CCR3273(a) was observed and cited.
