Employee's thumb amputated when caught by shaker machine

At approximately 6:50 a.m. on March 21, 1996, Employee #1, of the Teknor Apex Company, was operating shaker machine #1 in the compound department. His right thumb became caught in the nip point of the belt and pulley drive, and was amputated. The belt and pulley drive unit of machine was unguarded, and Employee #1 said he had fabricated a guard to use. The unit was within 7 feet of floor level.
