Investigation Summary

Investigation Nr: 170731905
Event: 08/25/1995
Employee burned from flash from heated vat of solvent

Employee #1 was performing a test on a valve under high temperatures. He heated a container with approximately 75 gallons of petroleum naphtha with an MSDS flash point of 104 degrees Fahrenheit and an autoignition point of 450 degrees. The laboratory analysis of this sample's flash point was 110 degrees. Employee #1 heated the container with 220 volt heating strips with a surface temperature of 725 degrees. The naphtha needed to be heated to 200 degrees to test the valve. The naphtha was heated for two hours, but it only got to 170 degrees. Employee #1 attempted to increase the temperature by tightening the wire straps that held the heat strips on the tank. The tank flashed and caught fire and the lid blew off the tank. Employee #1 suffered 1st- and 2nd- degree burns on his face.
