Employee suffers concussion when struck in head by plywood

At approximately 10:00 a.m. on March 22, 1994, Employee #1 was removing scaffolding. A plywood sheet, which measured 4 ft by 8 ft by 3/4 in., and which had been used in the concrete pouring of the beam soffit, was dislodged when another employee began climbing the scaffolding. The plywood fell approximately 45 ft through the scaffolding and struck Employee #1 in the head and upper torso. He was transported by paramedics to UCD Medical Center in Sacramento where he was admitted for 3 days for treatment of a concussion and three fractured ribs on his left side.
