At approximately 11:00 a.m. on January 24, 1994, Employee #1 was moving empty crates for soda products when he slipped on the floor, twisting and fracturing his right ankle. Employee #1 was transported by paramedics to Methodist Hospital in Sacramento, where he was admitted for treatment and released the following day.
