EMPLOYEE INJURED IN FALL FROM STRUCTURE

At approximately 10:45 a.m. on June 19, 1997, Employee #1 was removing (stripping) braces from the forms at an intake pipeline. Before the operation moved to the next area, Employee #1 used a ladder to access the top of the structure in order to move a fall arrest block from the north panel to the south panel. During the stripping operation, Employee #1 was wearing a full body harness with lanyards. However, during the time Employee #1 was on top of the structure, he was not effectively tied off. Employee #1 mistakenly stepped through a 22-in. gate opening at the top of the structure and fell 14 ft to a concrete sill. Employee #1 sustained a fractured left ankle in the fall. Employee #1 was hospitalized as a result of the accident.
