Investigation Summary

Investigation Nr: 170717292
Event: 04/29/1994
Employee's finger amputated in roller sprocket

Employee #1, of the Grandview Heights Citrus Association, was inspecting boxes of oranges and adding or turning oranges to make the top layer look good. The section of conveyor that she was working on was not designed to be used as a work station. The boxes of oranges were coming down the conveyor at a fast pace. At times, Employee #1 had to hold some boxes with one hand while adding and turning oranges with the other. Somehow, she placed her hand on the frame of the conveyor and let her fingers curl inside the frame where the roller end, chain, and sprockets are located. The rollers are connected to a chain that powers the conveyor. Employee #1's glove became caught by the chain and her hand was pulled into the sprocket of the roller. She required hospitalization for the amputation of her right index finger.
