Employee's leg fractured by push arms on log deck

Employee #1 is a lead millwright at a sawmill. He was made aware of a problem with the butterfly valve for the push arms on the infeed log loading deck. Employee #1 and the assistant maintenance manager locked out everything except the pneumatics, which operate the push arms, prior to starting work. Employee #1 got down in the operating zone of a push arm to use a 2 by 4 to move the butterfly valve linkage where he thought the problem might be. The area where he was standing was wet due to the watered logs, and he slipped and accidentally hit the butterfly valve with the 2 by 4. The butterfly valve caused the push arms to operate. Employee #1's left leg became caught in a scissor point between the push arm and the dead skid. He suffered a double compound fracture of the lower leg. The cause of the accident was failure to turn off and lock out the pneumatic power supply to the push arms. The employer was cited for a violation of T8CCR 3314(b), serious.
