Employee injured when forklift overturns

Employee #1, an equipment operator for an ice plant and fish processing operation, normally drives either an inside standup forklift or an outside forklift truck. At the time of the accident, he was using a Towmotor outside forklift with no name plate. He had just placed some empty baskets on the loading dock and was backing away while lowering the forks when the front wheel on the driver's side went up on a 35 degree slope running parallel to his path. The truck tipped over, catching Employee #1's feet under it and resulting in multiple fractures in both feet and a fractured wrist. The forklift truck had been transferred to this location about a month prior to the accident, but it had been involved in another nonaccident overturn on September 15, 1995. The employer determined that the large diameter tires raised the center of gravity and that this truck was unsuitable for this location. It was removed from service.
