Investigation Summary

Investigation Nr: 170628101
Event: 05/25/1994
Employee's finger amputated in feed rolls

Employee #1 was cleaning dried glue from the feed rolls on a finger joint machine. The rolls have to be moved to do this cleanup task. Employee #1 turned the rolls on and left them on. Her left middle finger became caught in the gears that turn the rolls, amputating it to the second joint. Employee #1 states that she was taught to clean the rolls in this manner. The rolls should be moved by jogging them, not by turning them on and leaving them on. Employee #1's training on cleanup and lockout procedures was incomplete.
