Investigation Nr: 170628556
Event: 11/02/1993
Employee's finger amputated due to inattention

Employee #1 was working as a filler operator on an olive canning line. She was at the full can palletizer, which picks up a full row of cans and puts it in a basket called a retort basket. Employee #1 had her hand on the side of the basket, which is constructed of expanded metal with 1-inch openings. Employee #1 states that she was not paying attention to her hand placement and did not realize that her third finger was sticking through the basket. Her fingertip was amputated when a row of 1-gallon cans was set in place and pinched off her finger.
