Investigation Summary
Employee #1, a sawmill worker, moved from his work station to another employee's work station to adjust a limit switch on a sorting chain. This chain moves lumber horizontally from one chain to another. Employee #1 did not lock out this machine before working on it. The machine did a complete rotation as Employee #1 reached in and touched the limit switch. Employee #1 was injured as a result. Employee #1 was doing a millwright's job, but had not been assigned to work at this position. The millwright was walking up to do the job when Employee #1 jumped in and did it.
