Two employees injured by falling pipe

Employees #1 and #2 were struck by a 42 inch diameter pipe when it fell from a raised position. The pipe was to be removed because it had a buckle in it. The pipe was raised for this repair and was resting on "rollers." A roller is a system used to move pipe using 4 aircraft tires fastened to a platform. The platform lies on the ground with the tires in an up position. The pipe rests on these tires as it is moved into position. One of the tires on this roller was flat. This roller needed to be removed. So Employees #1 and #2 were putting a cable on this roller so it could be pulled from underneath the pipe. The pipe fell when one or more of the slings supporting it failed. Employees #1 and #2, who were wearing safety caps, were struck by the pipe as it fell. The pipe fell on the rollers that supported it approximately 2 feet above the ground.
