Investigation Summary
Employee #1 was using a 6 in. air-powered grinding disc to clean the bottom of a mold. He was wearing leather gloves, a face shield, and a leather apron. He had been grinding for approximately 20 minutes when the wheel hit a crack and he lost control of the hand grinder. The wheel tore off his left glove and severely cut his wrist before he could release the ON and OFF trigger. Employee #1 was hospitalized for five days with ligament and nerve damage.
