Investigation Summary
Employee #1 was carrying buckets of hot asphalt on a roof that had a pitch less than 4:12 when he slipped on roof flax that had just been applied. When he was interviewed, he was unaware of the slipping hazard of roof flax. The application of the new roof covering was not well planned to keep employees from stepping on the flax. Employee #1's hands were burned by the asphalt. He was not wearing hand protection and was wearing a short-sleeve shirt.
