Employee seriously injured in fall from elevated platform

At about 10:00 p.m. on November 1, 1993, Employee #1 was working from an elevated platform applying masking film to some windows, in preparation for painting nearby areas. Somehow, Employee #1 fell 12 feet from the elevated position, either from the scissor lift platform or from a wall ledge. Employee #1 was seriously injured, requiring intensive hospital treatment. There were no witnesses to the event, and Employee #1 does not remember what happened.
