Investigation Summary

Investigation Nr: 170379069
Event: 02/12/1994
Employee's leg crushed between dolly and loader

A plane was being readied for departure and cargo in containers was being loaded into the forward and aft holds of the plane. The aft loading crew had finished loading, so the operator of the loader cleared the area and parked the loader between gates 80 and 82. The forward loading crew was running somewhat late because an LD-3 dolly was damaged when it crashed into their loader. The dolly was taken out of service, tagged, and towed by a cart driven by the runner of the forward loading crew, along with three other dollies, to the same parking area where the aft loader was parked. Employee #1, the aft operator, was still standing at the foot of the ladder of the loader when he noticed the runner drive past him. After greeting the runner, he turned to walk away from the loader. At that moment, the side of the damaged dolly crushed his leg against the frame of the loader. The runner and Employee #1 admitted to not having seen each other until the last minute. The runner also did not notice that the damaged dolly was not tracking properly, so did not realize that the dolly was about to crash against the other loader.
