Employee's foot run over by scissor lift

At approximately 2:10 p.m. on May 12, 1995, Employee #1, of the Royal Glass Company, was working with a construction crew to apply waterproofing materials to a building that was under construction. Employee #1 had been working from the platform of a 4-wheel drive scissor lift operated by the supervisor. As Employee #1 was climbing down from the platform after it was lowered, the supervisor backed the lift before Employee #1 got clear. The lift's wheel ran over his foot, crushing it severely. Employee #1 required hospitalization.
