Investigation Summary
At about 3:00 p.m. on May 10, 1995, Employee #1, of Loral-Randtron Systems, was retrieving an item from a storage shelf. To get to the shelf, she walked across a tiled floor that was wet from being cleaned just before her arrival. She slipped and fell on the wet tiles, fracturing her ankle. There was no warning of the slippery floor.
