Investigation Summary

Investigation Nr: 170379416
Event: 09/30/1994
Employees suffers amputation of fingers

When Employee #1, of Raychem Corp., utilized a tubing cutter that was provided by his employer to cut plastic tubing to size, the cut edges would occasionally be jagged. When Employee #1 (the operator) and a helper reworked the edges with utility knives, however, their hands were frequently cut by the knives. As an alternative, Employee #1 tried to rework the edges by feeding the pieces through the discharge end of the cutter. In order to reach the blade of the cutter, which operated with a strong motion, he had to enter the 32 in. by 15 in. opening until about half of his torso was in. While Employee #1 positioned a piece of tubing the machine cycled. The clamp caught his left hand and prevented him from pulling his hand from the path of the blade. Employee #1's fingers were amputated and he was hospitalized.
