Investigation Summary
Employee #1, who had been out with the flu, returned to re-roofing a 1-story, single family residence. Employee #1 was sitting on the 3 1/2:12 attached roof up 6 ft from its edge, applying composition shingles with an electric nail gun. For an unknown reason, Employee #1 fell 8 ft from the roof to the sidewalk. He suffered a concussion, fractured bones in his face, and broken teeth. There were no witnesses.
