Employee's fingertip severed in coffee machine cylinder

Employee #1 was cleaning an espresso coffee machine that had been on loan from a vendor. The employee was using a cleaning method that had been shown to her by her supervisor. Employee #1 inserted her fingers into the cylinder portion of the coffee machine, as she had seen her supervisor do. But the machine was still energized and was stalled in mid-cycle. The machine completed its cycle after Employee #1 inserted her finger, severing the middle fingertip of her right hand. The employee was caught in the machine until extracted by paramedics. To clean the machine, the supervisor had previously bypassed the safety device that disengaged power to the machine.
