Employee's finger skinned to bone by turning bar

Employee #1, an ultrasonic tester operating technician, was testing pipe seam welds for flaws when a crooked 36 in. diameter by 40 ft long pipe was brought into the test station to be straightened. Employee #1 used a 4 in. long turning bar to straighten the pipe. As he made the last pull on the bar to do the final straightening operation, the pipe kicked back and struck the turning bar. The turning bar hit the fixed frame of a newly-installed, automated ultrasonic testing machine, located 58 in. above and 15 in. behind the work platform. The impact pinched the flesh off the end of Employee #1's left index finger and removed it to the bone. Employee #1 was treated at a hospital emergency room and released. The emergency room physician listed the injury as an amputation, but no bone or fingernail was lost, only the fleshy finger pad of the first joint.
