Investigation Summary
Employee #1, of McCormick and Co., Inc., removed a bin access panel above a powered gate valve to unplug a hopper. He did not turn the machine off and did not lock out the controls. He was able to reach into the valve area to probe for bridging above the valve. His fingers on the right hand entered the valve and were immediately amputated. One of his fingers was amputated at the knuckle and the other at the nail. Surgery closed both fingers at the knuckle.
