Investigation Summary

Investigation Nr: 170377485
Event: 02/03/1995
Employee's eye injured when splashed by industrial soap

Employee #1 was pumping ZED Formula 50 (an industrial strength liquid soap) out of a 55 gallon container into a 5 gallon bucket in order to pour it into a floor cleaning machine to clean auto stalls. Some soap came out of a tube on the upstroke and splashed into his right eye. He was assisted by a coworker over to the emergency eye wash facility, where he flushed his eyes. He was also examined by a local doctor and released. He did not sustain any further recordable injury.
