Employee's thumb tip amputated by table saw

At approximately 12:30 p.m. on March 20, 1995, Employee #1 was using a 10 in. Black & Decker table saw to cut a 4 ft long by 3 1/2 in. wide piece of wood planking for hardwood flooring. When Employee #1 pulled the saw blade guard back to make an angle cut he accidentally ran his left hand into the saw blade. The tip of his left thumb was amputated and his index and middle fingers were lacerated. Employee #1 was transported to Kensington Hospital.
