Investigation Summary
At approximately 1:20 a.m. on June 8, 1985, Employee #1, an operator for a wire company, was working on a Morgan wire drawer #17. He had recently completed five weeks of training. The wire on the first block on wire drawer #17 became overloaded. The employee proceeded to use his 9 inch Kline side cutters to feed the wire, a procedure the company had trained him in. His gloved right little finger became caught under the wire on the block and pulled his fingers in, causing amputation to the first joint of his index finger and to the first joint of his middle finger. Employees should not reach any part of their body into the point of operation when proper guarding is provided by the employer.
