Employee's arm crushed when caught in conveyor system

At approximately 10:30 p.m. on July 27, 1995, Employee #1, a 19-year-old sorter/loader for a package delivery service, with over two years of employment at the site, was trying to remove a package from a moving conveyor system. He stood on the rear of a tug truck to reach into the system. He failed to observe the employer's written procedure to de-energize the system prior to attempting to remove packages, and his left lower arm was caught and crushed. It was later surgically amputated below the elbow.
