Employee injured in fall from operator's platform

Employee #1, a 45-year-old contract temporary warehouseman, had been working four months for Kao Infosystem Co., a wholesale distributor of computer software. At approximately 7:30 p.m. on February 15, 1995, he was instructed by his supervisor to replace a broken pallet using a Raymond electric order picker, model 152. As he moved closer to the rack, he lost his balance and fell approximately 18 ft from the operator's platform to the floor. Employee #1 was hospitalized with fractures of both femurs, his right elbow, and his left patella. He was not wearing a lanyard and safety belt.
