Employee killed in fall from unsecured ladder

At approximately 8:00 a.m. on February 6, 1995, Employee #1, a 46-year-old part-time roofer with more than 15 years of experience, was removing roof gutters from a single family home when he fell approximately 15 feet from a portable metal extension ladder. He died of skull and brain injuries. The employer, an unlicensed roofing contractor, failed to secure the ladder.
