Investigation Summary
At approximately 2:35 p.m. on September 8, 2003, Employee #1 was using a skill-saw to cut a 2-inch by 4-inch block of wood into shims. As he was operating the saw, Employee #1 used his left hand to hold the block of wood in place. For some reason, the saw kicked backward and his left hand came into contact with the blade. Employee #1 was hospitalized and treated for a partial amputation to his ring and middle fingers, as well lacerations to his left index finger.
