Accident Investigation Summary
Summary Nr: 202313466Event: 05/23/2003Employee Falls From Ladder
On May 23, 2003, Employee #1 was checking and recording facilities equipment into a system. He climbed up a fixed metal ladder to the scrubber unit. He stopped at the magnesium hydroxide tank. He removed the lid and checked twice inside the tank to record the fluid levels. Employee #1 fell approximately 11 feet from the ladder to the surface below. It is not clear why he fell. His Employer did not provide a platform and safe access to the tank. Employee #1 received a concussion, a laceration, and a fractured arm and wrist, and was taken to a local hospital for treatment. No witnesses observed the accident, and Employee #1 does not remember falling. --
