Investigation Summary
At approximately 4:30 a.m. on April 30, 2003, Employee #1, a lead sorter at a garbage collection and recycling material facility, was walking towards her sorter work station when she was struck from behind and dragged by a forklift. The forklift operator was driving a TCM forklift, Model Number FHG30N5T, forward with two bales of recycled material on the forks. The forklift operator's vision was blocked by material on the forks. Employee #1 was hospitalized and later died from multiple traumatic injuries.
