Investigation Summary
Employee #1 was operating a Rotoblast sand blasting machine table. He was standing in front of the machine loading and unloading parts from five circular auxiliary tables continuously transporting the parts to be cleaned through the point of operation (blast area). The procedure is to align two parts on the vertical slots on the portable plates. The plates are positioned in the center of the table and parts are transported through one cycle for cleaning and blasting. The sequenced is repeated after turning the part over. Preceding the second cycle, the employee removes the parts and places it on a pallet. While attempting to remove the second part, Employee #1 reached in with his right hand to remove the part and his right arm became caught by the revolving turnstile partition, pinning his arm between the partition and cabinet of the machine. He was hospitalized for his injuries.
