Investigation Summary
A crew of four employees was assigned to install 4-ft by 8-ft beams into an existing roof structure. They would remove the existing roofing material and 4 ft by 8 ft plywood and installed beams. While waiting for more beams to be brought to the jobsite, Employee #1 walked away from the unguarded roof opening and returned to the area. She fell approximately 18 feet through the roof opening onto the epoxy and concrete floor. She was hospitalized for her injuries.
