Employee amputates part of her hand on a table saw

On October 10, 2000, Employee #1, a store manager, who was both experience and qualified to operate a table saw, was cutting 1.25 in. strips of plastic. The guard was in place, but she was not using a push stick or other safety device. A sheriff report however, indicated that there was no guard. On the last strip to be cut, she suffered a partial amputation of the right hand and was hospitalized.
