Employee's fingers amputated while operating a forklift

Employee #1 was operating a Toyota forklift to push a 4-ft by 8-ft wood table into a Penske rental truck, using the mast. Employee #1 could not get the forklift inside the truck so he dismounted the forklift and pushed and pulled the table in. He mounted the forklift and put gear in reverse and pushed the accelerator pedal, but truck did not move. He attempted this maneuver again, and truck tilted, causing Employee #1 to slip off the chair. He reached up with his left hand and grabbed the metal overhead guard, but the forklift started to move. His fingers were caught between the metal overhead guard and the top inside frame of the truck. His index, middle, and ring fingers were amputated. He was hospitalized.
