Investigation Summary
Employee #1 and Employee #2 were attempting to splice together two pipes which were 16 inches in diameter and 10 ft long. They could not align the pipes properly and decided to lift the pipes onto the ground. While lifting a pipe with a metal C-clamp, the clamp slipped from the pipe. The pipe fell approximately 3 ft onto Employee #1 who then fell onto Employee #2 causing them both to fall to the ground. Employee #1 was hospitalized with a fractured shoulder. Employee #2 received bruises and contusions.
